- Veterinary health form confirming that it has been trained to behave properly in a public environment
- Confirmation of passenger responsibility and form regarding behavior of psychiatric / emotional support animal
- Letter from a licensed healthcare professional
- In the event of an animal weighing more than 65 pounds, the company will determine if it is fit for flight

- Minimum age: 4 months
- Ex: Assistance for people with visual impairments, deafness or epilepsy, and people with reduced mobility.
Health conditions
- Minimum age: 16 weeks
- The passenger must inform himself of the regulations of the countries of departure, transit, and arrival.
- Travel to the continental United States:
- The passenger must have a health certificate and proof of vaccination against rabies.
- Owner’s name, phone number and address
- Information relating to the identification of the animal, including breed, sex, age, color and spots
- The date of vaccination and the name of the vaccine administered;
- The date the vaccine expires. If no expiration date is indicated, vaccination must have been completed within 12 months of arrival in the United States.
- After the rabies vaccine, it is not possible to travel within 30 days of vaccination
- The passenger must have a health certificate and proof of vaccination against rabies.

The trip
Container location
The container should be placed under the seat in front of the passenger.
Passenger location
The passenger must not sit in front of a partition or in a row of access to an emergency exit.
The passenger must not sit in a United Premium
Plus place seat.
Pets are not allowed in premium cabins on board Boeing 757, 767, 777 and 787 United Economy customers on board a Boeing 757-200 must sit in a window seat.
The tariff
125 USD per way.
If a stopover lasts more than 4 hours in the United States, or more than 24 hours outside the United States, an additional charge of 125 USD will be required.
